I have read all of the information provided with this application and on the Booth Space Information Page. I agree to abide by all the rules described therein and release Madison Chautauqua and the Madison Area Convention and Visitors Bureau from any and all liability for any damage, injury or loss to any person or goods which may arise. I understand no refunds will be made after July 15, 2008 and my jury fee of $10.00 is non-refundable. A $25.00 cancellation fee will be charged for refunds prior to the July 15th deadline. If for any reason I cannot attend the Madison Chautauqua after my entry is accepted, I must notify MACVB in writing or by telephone before the show opens, if I wish to be invited back. I must be present with my work throughout the show and cannot use a representative in my place to attend the show.

I have included with this application a cover letter, brief history, slides (a sample if necessary), a business size #10 SASE with sufficient postage for return of the slides and  a check for deposit of $100.00 plus $10.00 for the non-refundable jury fee.  Balance due upon acceptance.
